Medical Records Request

For medical records from Truckee Meadows Fire & Rescue, please send a secure email to emsdivision@tmfpd.us.

In order for us to serve your needs, please provide the following required information:

Requestor Information:

1. Requester name

2. Agency/organizations represented

3. Contact information to include a phone number and email address

Patient Information:

1. Patient Name

2. Date of incident

3. Incident Location or Address

4. Date of Birth

Please Allow Up To 30 Days For Us To Provide Medical Records.

HIPAA signed releases must be included in any medical records request. If you do not have a HIPAA release form, download the form below and submit to emsdivision@tmfpd.us, or fax at 775.326.6003, or mail to TMFPD EMS Division, 3663 Barron Way, Reno, NV 89511.

Records released to a patient must be picked up from our office. Identification must match the release form.

Billing Information is also included on the form that can be downloaded.
